• What does this service provide for palliative care patients and their carers?

The ward team will look after your day to day care, including managing your symptoms and arranging your discharge.  Outside of normal working hours a consultant in palliative medicine will be available to give advice to the ward team if they request it.  They work with the doctors and nurses to improve your physical comfort (such as treatment for pain or sickness) and to assist them with decisions about your future treatment.

You need to be referred by a doctor or nurse on the ward where you are having treatment.

You will be given specialist palliative care if you are an inpatient at Leeds General Infirmary or St James’s University Hospital.  Your home address does not matter.

The District Nursing Night Service is available from 9pm to 7.15am

• What does this service provide for palliative care patients and their carers?

You will receive care from district nurses, community staff nurses or community nursing assistants.

A qualified nurse will assess your overnight care needs and your district nurse will arrange your overnight visits.

A community nursing assistant will care for you in your own home from 10pm to 7am.  The nursing assistant can contact a qualified nurse for advice or assistance at any time.

If you need extra help a qualified nurse is available to visit.  They can help in many ways such as helping to manage your symptoms, assisting with any catheter problems or pressure relief.  Requests can be made by patients, carers or your GP.

• How do patients and carers access this service?

The service covers the whole of the area covered by Leeds City Council.  If you feel you would benefit from this service please discuss with this with your district nurse.  They can request overnight care or plan a visit during the night.

If your district nurse registers you for palliative care services you will be given a priority phone number.  This number will be displayed on a pink sticker on your district nursing records.  You should ring this number for assistance out of office hours.

Outside of office hours you, your carer or your health care professional can call the Nurse in Charge on 0113 218 5511 for advice and information.  If you are an inpatient you will receive 24 hour care at the hospice.    To receive our other services during the day you will need to be referred to the hospice by a health care professional.

If you have a life limiting illness St Gemma’s Hospice can give you specialist palliative care and support your family and carers.  We provide care through our Inpatient Unit, Day Services and the Community Team.  We can help you with symptom management, emotional and spiritual care, rehabilitation, terminal care and bereavement support.

This line is answered from 6.30pm to 8am Monday to Thursday and from 6.30pm on Friday over the weekend until 8am on Monday.
