• What does this service provide for palliative care patients and their carers?

The ward team are responsible for managing your day to day care, including management of symptoms and arranging discharge. However occasionally it is necessary for the ward team to access specialist palliative care advice out of normal working hours (when the specialist palliative care team are not in the hospital) and in this instance a consultant in palliative medicine will give advice to your ward team.

A 24 hour service is provided by the In-Patient Unit. Community Team operates Monday to Friday 8.30am to 5pm. Outside these hours, telephone advice/information is available for patients, carers and health care professionals by contacting St Gemma’s Nurse in Charge on 0113 218 5511. Medical advice can also be accessed through the Nurse in Charge.

• What does this service provide for palliative care patients and their carers?

St Gemma’s Hospice provides specialist palliative care for people with life threatening illness and support for their families and carers. Care is provided via the In-Patient Unit, Day Services and community team. Services provided include symptom management, psycho-spiritual care, rehabilitation, terminal care and bereavement support.

Nursing Service: 9pm to 7.15 am with calls being triaged from 6.30am to 7.15am

• What does this service provide for palliative care patients and their carers?

Provides overnight care from 10pm to 7am by a Nursing Assistant on a one to one basis in the patients own home. A supervisory visit is carried out during these hours by a qualified nurse, who can also be contacted by the Nursing Assistant at any time during the night for advice or assistance.

The qualified nurse is available to attend to requests for crisis intervention visits. These requests may be made by the patient/carer/GP.

A team of two Nursing Assistants are available to provide visits during the night for those patients who require care but not a full night of overnight care.

During office hours referrals are made by the Caseload Manager.

Between the hours of 9pm and 6.30am the team are contactable on 0845 120 7067 or the direct palliative care line, their number of which is on the SAP documentation within the District Nursing notes if this number has been given.

Areas covered by our service are primarily dependant on location of the person’s G.P. and mainly cover the eastern part of Leeds.

There have been significant changes to the Out Of Hours service in West Yorkshire.  It is now known as West Yorkshire Urgent Care Service.

This service is provided by:

• NHS Direct (access and assess)
• Local Care Direct (treatment in Bradford, Leeds, Kirklees and Wakefield)
• Care UK (treatment in Calderdale)

If your district nurse registers you for palliative care services you will be given a priority phone number.  This number will be displayed on a pink sticker on your district nursing records.  You should ring this number for assistance out of office hours.

This line is answered from 6.30pm to 8am Monday to Thursday and from 6.30pm on Friday over the weekend until 8am on Monday.

Your details will be recorded by a health advisor.  This information will then be assessed and prioritised by a GP Advisor.  If you require face-to-face treatment you will either be given an out-of-hours home visit or referred to your GP as appropriate.
